This solution helps Care Coordinators monitor patients preparing for a major surgical procedure.
When patients skip pre-op tasks it puts the surgery in jeopardy. Here the CC can set up Pre and Post-op checklists for each patient and monitor their progress as they complete tasks.
The experience starts with a patient list
Clicking on a patient shows this view, where they can start a checklist
They select tasks for the patient to do and tailor it as needed
The CC can easily see the patient's progress and follow-up if needed
The CC can send a push notification to the patient to make sure they complete a critical task in time
The process is similar with Post-Op tasks, except the tasks are different and usually performed every day
Patient-Facing Product Design
The patient-facing app is easy to use and performed very well with patients in testing.
The patient will see the checklists that the Care Coordinator sets up for them. Likewise, as the patient checks-off tasks and medications, the CC sees that progress.
The home screen makes it easy to see medication and checklist progress
The patient can quickly mark tasks as completed and see more details on any task
The patient can respond if the Care Coordinator reaches out. As they make progress they can see what their journey looks like
